DEPOSIT REQUIRED:

◦ A $50 commitment fee is required to confirm your set appointment date. This goes towards the total balance due at the time of service via Zelle, Venmo, Apple Pay, cash or card.

◦ Once deposit is sent, you will receive a text confirmation right away as well as 24 hours prior to your appointment as a friendly reminder.

RESCHEDULING:

◦ We understand life happens! If you need to reschedule, please try to let us know at least 48 hours in advance.

◦ No shows will automatically forfeit deposit and be charged remaining balance of service.

Your safety is our #1 priority. Those who have the following will be ineligible for permanent makeup:

• Under the age of 18

• Actively Positive with Covid

• Pregnant or nursing

• Diabetic (insulin dependent)

• Actively on anticoagulants, blood thinners, or long term use of Aspirin, Tylenol, and/or Ibuprofen

• Autoimmune Disease

• Glaucoma

• Hyper pigmentation or hypo pigmentation

• Blood disorders

• Heart conditions

• Individuals with conditions such as blood clotting, immunodeficiency, lupus, or keloid scarring

• Allergies to Lidocaine or Epinephrine

• Post inflammatory hyperpigmentation

• Use of Accutane / Retin A within the last year

• Use of Latisse or any other eyelash/brow growth serums within 6 months

• Allergies to metal and/or color

• Poor reactions to prior tattoos

• Open wounds, rashes, and/or any skin conditions on the procedure area

• Epilepsy (severe and prone to fainting)

• Allergies to pigments or topical anesthetics

• Clients who have undergone organ transplant

• Clients with skin conditions (Eczema, Dermatitis, Rosacea, Psoriasis, etc)

• Individuals who are undergoing or scheduled for Chemo/Radiation Therapy
